Nomination Forms
We encourage nominators to use the online nomination tool to submit nominations. The online form now makes it possible to save a nomination in process and return to it later for completion and submission.
Click on any of the links below to download a nomination form in Word format. Please save the form to your desktop before filling it out. Completed forms should be sent via email to distinctionawards@tufts.edu or submitted via campus mail by February 26 to Tufts Distinction Awards, c/o HR Vice President’s Office, Human Resources, 200 Boston Avenue, Suite 1600, Medford, MA 02155. You can also fax your nomination with a cover sheet to 617.627.3725 (attention: Jackie Foster, Nomination).
Paper copies of the nomination forms are also available at all the Tufts Human Resources offices for those without computer access.
